Do you have a health condition that makes it difficult for you to serve in your assigned state? If yes, then you might be interested in applying for NYSC relocation on health grounds.

NYSC relocation on health grounds is one of the ways that corps members can request for a change of state after being deployed by the NYSC management.

This method of relocation is based on the medical report that shows the critical state of your health and why you need to serve in a different state.

In this blog post, I will show you how to apply for NYSC relocation on health grounds, what are the requirements, and what are the chances of getting approved.

How to Apply for NYSC Relocation on Health Grounds

There are two ways to apply for NYSC relocation on health grounds: in-camp and online.

In-camp application: This is the application that you make during the orientation camp.You will need to follow these steps:

• Request and obtain a relocation form (to be available in camp after the opening ceremony)
•  Attach a written letter. (The format of this letter is usually available in camp by officials).
• Attach your genuine medical report legally issued by a certified medical practitioner (Doctor’s Report).
• Tender it to the officials in charge.
• Expect the result on the last day of camp after the interview (best of luck).

Online application: This is the application that you make after leaving the orientation camp. You will need to follow these steps:

• Visit the official NYSC portal via https://portal.nysc.org.ng
•  Enter your email address and password to login to your NYSC dashboard
• On the portal, scroll to “Relocation” and click on it
• Fill in the required details and upload your medical report and other supporting documents
• Submit your application and wait for approval

Note that online application is only available within 12 months after leaving the orientation camp.

Requirements for NYSC Relocation on Health Grounds

The main requirement for NYSC relocation on health grounds is a genuine medical report that shows your health condition and why you need to relocate.

The medical report must be issued by a certified medical practitioner from a reputable hospital or clinic. The report must also be stamped and signed by the doctor.

The medical report should contain the following information:

• Your name, date of birth, and state code
•  The name, address, and registration number of the hospital or clinic
• The name, qualification, and signature of the doctor
• The diagnosis, prognosis, and treatment of your health condition
• The recommendation for relocation and the preferred state

Some examples of health conditions that can qualify you for relocation are:

• Asthma
• Sickle cell anemia
• Diabetes
• Hypertension
• Heart disease
• Kidney disease
• Epilepsy
• Cancer
• HIV/AIDS

Note that not all health conditions are eligible for relocation. Some conditions that are not accepted are:

• Malaria
• Typhoid
• Common cold
• Headache
• Dysmenorrhea
• Pregnancy

• The severity and genuineness of your health condition

Sir/Madam,

I am writing to request for a relocation from my current state of deployment, Kano, to Lagos state on health grounds. I am a Batch B Stream 2 corps member with the NYSC code LA/21B/1234.

I have been diagnosed with sickle cell anemia, a genetic blood disorder that causes severe pain, infections, and organ damage. I have been managing this condition since childhood with regular medications and blood transfusions.

However, the harsh weather and living conditions in Kano have aggravated my symptoms and made it difficult for me to cope with the demands of the service year.

I have attached my medical reports from the Federal Medical Centre, Kano, and my previous hospital, Lagos University Teaching Hospital, as evidence of my condition and treatment history.

I also have a letter of recommendation from my doctor, Dr. Adeola Ogunbiyi, who advised me to relocate to a more conducive environment for my health.

I humbly request that you consider my application and grant me a relocation to Lagos state, where I have access to better medical facilities and support from my family.
